New patients are always welcome at Smiles Unlimited. We strive to provide our patients with the highest standards and quality available. Patient comfort and care combined with professional, state of the art dental services is what sets us apart.
We advise new patients to arrive about 10 minutes before your appointment to finalise your new patient paperwork. Please note that any appointment changes or cancelations must be advised at least 24 hours before your scheduled appointment.
If you have hospital vouchers, please bring them along to your appointment if required.
Below is a copy of our Patient Medical History Form for your convenience. If you have a minute, please either fill in the online form and submit or download the PDF form, print, complete, and bring with you on your first visit, but don’t worry, you will also be able to do this on the day of your appointment.
